Medicaid is a source of health insurance coverage for Utah's vulnerable populations. Medicaid is a state/federal program that pays for medical services for low-income pregnant women, children, individuals who are elderly or have a disability, parents and women with breast or cervical cancer. To qualify, these individuals must meet income and other eligibility requirements. Provides Medicaid plan resources and provider information for SelectHealth Community Care members.

0 - 18 years of age, children and adolescents with special health care needs in Utah. No fee.

Assists families of children and youth who have special health care needs with coordinated care planning, education and resources, in order for them to make informed decisions. This may include primary and special health care, behavioral health, developmental and educational programs, financial support resources and social services that meet their special needs from infancy through the transition to adulthood.

Services Include:

• Transition to Adulthood website provides age-specific recommendations and community resources for any Utah resident age 12-22 who requests assistance with transition preparation and readiness.
• Technology Loan Service- lends Chromebooks and hotspots to families of children with special health care needs. Available at locations in Blanding, Richfield, Vernal, Moab, Price, Ogden and Salt Lake City.

Individuals who reside in Utah and are eligible for Traditional Medicaid Services, who do not have transportation to medical appointments.

Provides non-emergency transportation options for eligible Medicaid members to assist with transportation to medical appointments.

UTA Bus - for those with Traditional Medicaid and who live in an area served by UTA or Cedar Area Transportation Services (CATS). Contact a Medicaid eligibility worker for more information. 

Para-Transit Bus Services are available in some areas. Contact Medicaid at 801- 538-6155 or toll-free at 1-800-662-9651 for FlexTrans vouchers.

ModivCare (formerly LogistiCare) - for those with no other way to get to Medicaid covered services. Call 1-855-563-4403 three business days in advance to set up your ride. Have the following ready when calling:

• Name and birth date of the patient
• Medicaid ID
• Address and phone number of picked up location
• Name, address and phone number of Medicaid medical provider being seen
• Date and time of the appointment
• Will an attendant be riding also?
• Can the patient walk unaided? If not, what type of aid will be used?

Primary focus is on families of Jordan School District, but will assist anyone in need.

Increases awareness of the need for health insurance for all students and assists with applications for Medicaid and CHIP. Other services include general health insurance information, navigator programs, and specialized information and referral.COVID-19 UPDATE: Please call to speak with an employee. There is a limited number of visitors at the office. Trying to accommodate clients by seeing them at other locations (such as schools) or having the clients stay in the car while filling out application information that employees will then fax in. Available for children with multiple, complex chronic illnesses. A medical team evaluates kids ages 0 to 17 for program enrollment. Any insurance provider accepted. Referrals welcome. Salt Lake County, southern portion of Davis County.

Connector Service assists patients and families with scheduling and support services, including home visits by a registered nurse. Three or more chronic illnesses, such as: cerebral palsy, neurologic impairments, feeding issues, or vision/hearing impairment. A multi-disciplinary team assists children who have multiple, complex chronic illnesses. Interventions, including home visitation, help support the family so children can grow healthy in their home environments. Facilitating communication among providers is an essential objective. Shared Plan of Care.
