A state operated Intermediate Care Facility, providing an array of resources and supports. Therapeutic, clinical and specialty services. Provides 24-hour-per-day services as well as a day program. Limited in-home respite services. 95% of the USDC is an adult population. The majority reside in our residential buildings. Clients each live in an apartment with four to six others. With staff help, they shop for groceries, cook their meals, and clean their living areas. Therapies include: speech services, nursing services, physical therapy, recreation services.

Mission at West Jordan is a non-profit organization providing residential treatment to adults with intellectual and developmental disabilities. Services include skilled nursing (intermediate care nursing), habilitation and rehabilitation, assisted living, and care for the intellectually disabled. It also provides work placement, day treatment, school, or other appropriate daytime activities. COVID-19 Update: for the safety of vulnerable residents, no visitors are being admitted into any of our communities, only healthcare personnel, and community outings as well as outside entertainment are restricted.

18+ years old, have a cognitive or developmental disability, able to take care of their own personal hygiene and able to transfer to a standard van or vehicle. Accepts private pay, SSI, and some insurances. Cannot accept DSPD funds.

Day center where adults with special needs can socialize, develop, practice, and maintain life skills. Provides meaningful activities and nurtures lifelong relationships.
