Youth and adults with rheumatic disease

Serves children and adolescents with chronic, incurable autoimmune diseases who will require close and ongoing care over the course of a lifetime. Preparations for transition are formalized, streamlined, and enhanced in order to facilitate successful transfer to an adult rheumatologist.
